ENDOCARDITES À HÉMOCULTURES NÉGATIVES

Agents pathogènes possibles et moyens diagnostiques :

Coxiella burnetii (fièvre Q) - Sérologie IgG phase I > 1/800

Bartonella spp - Sérologie, culture cellulaire, PCR

Brucella spp - Hémocultures, sérologie

Tropheryma whipplei - PCR sur prélèvements tissulaires

Legionella spp - Sérologie, hémocultures

TRAITEMENTS DES ENDOCARDITES À HÉMOCULTURES NÉGATIVES

Brucella spp	Doxycycline 200mg/j + Rifampicine 600 mg/j + Cotrimoxazole	3-6 mois
Coxiella burnetii	Doxycycline 200 mg/j + Hydroxychloroquine 200-600 mg/j	18 mois ou plus
Bartonella spp	Doxycycline 200 mg/j + Gentamicine 3 mg/j	4 semaines + 2 semaines
Tropheryma whipplei	Doxycycline 200 mg/j + Hydroxychloroquine 200-600 mg/j	18 mois ou plus
Legionella spp	Lévofloxacine 1g/j ou Clarithromycine 1g/j + Rifampicine	6 semaines ou plus

MODALITÉS DE PRESCRIPTION

Posologies recommandées :

Molécule	Modalités
Ampicilline	200 mg/kg/j soit 2 g toutes les 4 h en IVD
Céfazoline	150 mg/kg/j soit 2 g toutes les 8 h en IVD
Ceftriaxone	2 g par 24 h en IVD (Streptocoque) 2 g toutes les 12h en IVD (Entérocoque)
Oxacilline	200 mg/kg/j soit 2 g toutes les 4 h en IVD
Gentamicine	3-6 mg/kg/j (8-10 mg/kg/j si état septique grave) en DUI
Rifampicine	20-30 mg/kg/j 900mg (<70 kg) ou 1200mg (>70kg) en 1-2 prises
Vancomycine	Dose de charge 15mg/kg sur 1h puis 30mg/kg/24h en continu
